Blazers will have to defeat Celtics in the preliminary final on Thursday night if they are to have a chance of meeting Home Boiz again this season and avenging the loss.
Leading 17-11 at half-time, Home Boiz dropped their guard in the third quarter as Blazers piled on 20 points, but the team quickly reversed the momentum of the game when they landed 20 points of their own in the final period to eke out a three-point win.
Scoring was shared around by Blazers with Jade Geary (13 points) and Leslie Dempsey (12 points) leading the way, while Paul Scarsi dominated the key for the victors with 15 points.
In the second A men's game, Celtics ended the season for Juniors with a 53-35 win.
Hoani Walker-Tepania was devastating under the ring for Celtics with 30 points and Jericho Young was in great touch for Juniors with 19 points.
Young's bag of points included five three-pointers, including one shot from well behind the three-point line.
Blazers fired in the final period in their B men's game against Kinetic Men 2 to book the first grand final spot with a 35-32 win, while Kinetic Men 2 will go head-to-head with Mavericks on Thursday to decide the second spot in the big game.
In a fast, close game, the scores were locked at 25-25 at three-quarter time before the Blazers defenders muscled up to restrict their opponents to seven fourth-quarter points.
Jericho Young was the best on the scoreboard for Kinetic Men 2 with nine points in a team performance inside the key.
Michael Parker (14 points) and Gary-Luke Naughton (10 points) led the way with the ball for the victors.
A 16-point haul by Patrick Caseres helped Mavericks to a 48-40 victory against Team Pilipinas in their B men's clash.
In a point-for-point final period, both teams added 13 points to the scoreboard, meaning it was Mavericks' 35-27 three-quarter time lead that secured victory in an entertaining game of basketball.
