The woman, Talieha, was moved to a unit by herself at community centre at Pine Rivers in January and committed suicide three months later.

Reports of the incident have prompted parents of other former patients to question the care provided to their children since the state government closed the Barrett centre, without providing an alternative facility.

Mr Springborg told the Queensland Budget Estimates Hearings in Brisbane on Tuesday that the young lady’s death in care would be investigated by a “root cause analysis” and then a coroner’s inquiry.

“We have no issue with the coronial inquest,” Mr Springborg said.

“The coroner is an independent person. The information will be made available to them and they will consider that based on the evidence within that root cause analysis,” he said.

A "root cause analysis"  is health industry terminology for an investigation into the reasons why an incident took place.

The Barrett centre was closed in January and families of patients were promised that equivalent community mental health services would be provided closer to the patients’ homes.

Parents say this has not happened.

Of the 10 final patients to be placed in community care since January, two have committed suicide.

Five parents have contacted Fairfax Media since Tuesday morning to say their children – all former patients at the Barrett centre – are not receiving the equivalent care.

Parents who supported keeping the centre open praised its links with a school run by the Education Department on the site. The school has been closed and shifted to Yeronga.

Other parents – who are not upset the Barrett centre is closed - are disappointed no consistent, alternative mental health care has been provided.

In September 2013, Mr Springborg promised Barrett Centre patients would be treated at other centres when the centre closed in January.

"Residents at the Barrett Centre will continue to receive the care they need," Mr Springborg said.

"In some instances, that might need to occur in adolescent care facilities in other hospital and health services," he said.

The family of 18-year-old Talieha have asked for a coronial inquiry into her April suicide death, after she was transferred from the 24-hour-a day care at the Barrett centre.

“She was living by herself in a little unit within the Pine Rivers Community Care centre,” her family said on Tuesday.

Mr Springborg  repeated the assurance on Wednesday care would be provided when he was questioned over Talieha’s death at Queensland Budget estimates hearings.

“If you look at the best advice available to me and others, young people should be actually supervised, if need be, on a 24-hour basis in the community with the support and in many cases close to their own family,” he said.

“We had adolescents that were transferred from right around Queensland, as far away as Cairns and beyond, to this particular facility and we are now trying to accommodate them in their own environment.”

However, a Cairns-based mother of a 17-year-old girl who spent 12 months at the Barrett centre before it closed - after a complex, tragic and disappointing series of treatments at adult mental health centres - said care was simply not immediately provided to her daughter.

Since then, it has been sporadic.

“She came back to Cairns - where services had not improved since February 2013 - and where we went without support for nearly a month,” the girl’s mother said.

“She went from having 24-hour-support in a specialised service, to home with nothing.”

Eventually care was found through HeadSpace – but that care finished on June 30 when funding ran out.

“I couldn’t believe that we would go from 24/7 intensive support to nothing in four months,” the mother said.

Her daughter now receives 12 hours' care a week, after the woman made a panicked phone call to Sharon Kelly, director of West Moreton Health’s Mental Health unit, which managed the Barrett centre.

The woman presented a parent’s alternative care model – on behalf of Barrett centre parents - to Premier Campbell Newman and his wife Lisa in October 2013, when Community Cabinet met in Cairns.

That report included a reference to a promise from West Moreton Hospital chief executive Lesley Dwyer that the Barrett would stay open until suitable, appropriate and effective Tier 3 placements could be found for all adolescents at the Barrett.

“That was told to me in person by Lesley Dwyer, CEO of West Moreton Hospital,” said the mother.

Adolescent mental health is now run by Queensland Health’s comparatively new body, Children’s Health Queensland.

The issue, “Discussion on the Adolescent Mental Health Extended Treatment Initiative and future reporting requirements” was raised at the board meeting on Children Health Queensland on March 27, although no details on replacement facilities were included in the minutes.

A new adolescent and child mental health unit is included in the new Lady Cilento Children’s Hospital soon to open at South Brisbane.

Mr Springborg said it was wrong to jump to conclusions before any coroner’s inquiry was finished.

Parents and supporters criticised Mr Springborg’s comments made at the Budget Estimates session on Tuesday about the number of “attempted suicides” at the Barrett centre.

“Of course a mental health facility for adolescents with severe and complex mental health issues is going to have suicide attempts,” one said.

“The fact that they were only ‘attempts’ illustrates that Barrett saved their lives.”
