Queensland's mental health commissioner says there appears to be failings in the way police respond to violent confrontations, following a week where three people were shot dead by officers.

In all three cases, police say the men rushed at them with a weapon before officers opened fire.
The incidents led Police Commissioner Ian Stewart to announce a review, along with consultations with health bodies and to make extra police resources available to the coroner investigating the shootings.
Queensland mental health commissioner Dr Lesley van Schoubroeck said she would meet with Mr Stewart on Tuesday to discuss preventing further deaths.
"I particularly acknowledge the need for de-escalation strategies to be the primary response in high risk interactions such as these," she said.
"While there is general recognition that police need to conduct their duties protecting the public in safety, and to go home to their families at the end of the day, it appears that something is failing and a systemic review may be required."
Mr van Schoubroeck said the shootings prompted the Mental Health Commission to fast-track studies into the interactions between people with a mental illness, drug and alcohol issues with the justice system.
She said they would also bring forward work to improve the relationship between mental health and justice systems.
Monday night's Gold Coast shooting was the sixth time this year someone in South East Queensland was hit by bullets fired by officers - four of which have been fatal.
Mr Stewart said there had been 13 police shootings in Queensland since 2010, with six people killed.
While not commenting specifically on this week's shootings, Mr Stewart said there were times when police were not able to use Tasers to subdue offenders.
"If someone's running at you with a knife and they're two metres away, trying to shoot them with a taser is not the sensible option," he said.
A man in his 30s was shot dead on the Gold Coast on Monday night after allegedly charging at them with a knife. A day earlier, Edward Wayne Logan was killed on the Sunshine Coast after allegedly wielding a weapon at two police called to a disturbance.
On November 18, Laval Zimmer was also killed when he confronted officers with a knife at a Kippa-Ring home. The other recent fatal shooting was that of Shaun Kumeroa, who was involved in an armed stand-off with police for several hours before he leapt out of a car and tried to point a gun at SERT officers.
Mr Stewart urged his officers to remain confident in their training, and asked the public to keep faith in the police.
"I have confidence in them. I trust them. I believe that they have good training. I believe that they honestly go out there with the best intentions on behalf of the community every day," Mr Stewart said.
Mr Stewart said police would first mount an internal review into its use of force model, before consulting with local and international experts.
"There are a number of people we believe come from policing academia who have a significant interest in this area and we will reach out to them," he said.
