The 52-year-old victim was left with a critical head injury after a 36-year-old man allegedly stabbed him during a street fight in the western Cairns suburb of Manoora about 2am on Wednesday.
A man from Gordonvale has been charged with grievous bodily harm.
The victim was rushed to Cairns Base Hospital before being transferred to Townsville Hospital for more treatment.
The 36-year-old man is due to appear in Cairns Magistrates Court on Thursday.
Police say the men know each other.
