Cronulla forward David Fifita was banned for a year in 2011 for abusing match officials in almost identical circumstances that sees him facing a lengthy ban from rugby league following an incident last Saturday.

Fairfax Media can reveal Fifita was suspended from the Penrith junior competition for one year in 2011 for abusing officials while acting as an on-field trainer for St Patrick's.

Alarmingly, he and twin brother Andrew were on Monday morning slapped with a similar breach for allegedly abusing and threatening officials after the A-grade match between St Pat's and St Mary's on Saturday.

David would have been facing a much harsher penalty - some officials have suggested a lifetime ban - if this latest breach had occurred within three years of the 2011 suspension.

But a three-year period without incident means he has been judged as a "clean skin".

News Corp reported on Monday that the twin brothers had been involved in an incident from August, 19, 2013, during a match between St Patrick's and Emu Plains.

The judiciary records for both brothers also reveal that they were suspended for six weeks in 2007 for "behaving in contrary to the true spirit to the game". They were 18-years-old at the time.

The brothers have five days to respond to the breach notices issued by the Penrith league, but of greater concern is what it means for their NRL futures.

It has been confirmed that there is no video evidence from last Saturday's incident at Meurants Lane, in which the brothers allegedly verbally attacked referee Tim Hannon and touch judges Cameron Turner and Nathan Sharp.

Feeling threatened, Hannon angered Andrew Fifita when he produced his mobile phone near the timekeeper's table after the match.

He has alleged in his incident report that the Kangaroos and NSW prop said: "Delete that now or I'll smash you".

It is understood that Hannon - a police officer - has gone to ground since the incident, while Penrith referees boss Phil Haines confirmed on Sky Sports Radio that one touch judge has refused to officiate A-grade matches again because the incident occurred in front of his young son.

Fairfax Media has obtained a full copy of Hannon's report, and the allegations are damning.

He alleges that David Fifita approached him after the match.

"David Fifita said to me, 'Hey, ref can I ask you a question?," the report says. "I said 'No, mate. You can't'.

"David said, 'You're a f--king disgrace mate. I lost respect for you when you didn't stop time when one of our players were down. You're a f--king disgrace' … The male ground manager, who after the game informed me he was the treasurer for the St Pat's club verbally, asked David Fifita to leave however he continually verbally abused me.

"We stood there for some 20 seconds with David Fifita still verbally abusing my refereeing performance. I said, 'Ok if you aren't going to do anything about it I'll pull my phone out and start recording this so it can be forwarded to match review committee'. I pulled out my iPhone and held it upright in front of my chest. I started recording the incident.

He alleges that Andrew Fifita then "charged towards me and attempted to snatch my mobile phone which I was holding in my right hand. Numerous people intervened and held Andrew Fifita back preventing him from grabbing my mobile phone. At the time these actions by Andrew Fifita made me feel very threatened and uncomfortable. I observed a male St Mary's official walk over and say to the male St Pat's ground manager, 'The ref's in the cops tell everyone to pull their head in'."

Hannon said that some officials asked him to delete the video, with a female ground manager asking: "Look can you just delete that video and not report it come on let's not make a big deal out of it. He (Andrew Fifita) has a lot to lose".
