Judge died in January on the day he would have turned 58, after a long battle with illness. He was a premiership player at Hawthorn in 1983, a star for East Fremantle in the WAFL and coached both West Coast and Hawthorn, taking the Hawks to the finals in his first year in charge.
It has been customary for the Hall of Fame night to honour figures within the game who have died in the past year - notably slain Adelaide coach Phil Wash this year - but Judge was missing from the 2016 video.
Radio 6PR Afternoons host Peter Bell, inducted into the Hall of Fame in 2015 after a career at North Melbourne and Fremantle that included two premierships, said he was sure the omission would have been accidental.
"I don't recall seeing Ken Judge on there," Bell said of the video.
"I can only assume it was an oversight."
Tuesday night's function saw South Fremantle and Richmond champion Maurice Rioli inducted into the Hall of Fame, alongside East Fremantle star Ray Sorrell.
The AFL has been contacted for comment.
